一、出境人员体检：
1. 体检时间：周一到周五上午8:00-12:00，前台受理报检时间8:00-11:30。
2. 所需资料：身份证或护照原件；近期2寸白底彩色免冠证件照4张。
3. 申办方式：在陕西国际旅行卫生保健中心官网（www.sxithc.com）、微信公众号（SNITHC）
或微信二维码预约办理，无预约不能体检。

4.取证：按照取证凭单上的时间在一楼大厅取证，可委托他人代为取证。
5. 申请免费传染病监测人员需提供的资料：
♦ 留学生：提供一年及以上签证、录取通知书、一年及以上I20表（仅是留学美国学生提供）的复印件。
♦ 访学：提供留学基金委一年及以上资助函复印件。
♦ 劳务：一年及以上签证复印件，本次出境一年及以上的劳务证明。
注：1）体检和接种疫苗可以同一天完成。
2）目前陕西保健中心没有相关海事部门授权，不能进行船员传染病监测体检，只能进行常规的
出入境传染病监测体检，办理《国际旅行健康检查证明书》，有效期为一年，且不包含甲肝抗体、咽
拭子培养及粪便培养等检查，毒品测试中不包含苯环已哌啶的测试。
二、预防接种：

2）需要翻译计划免疫接种记录的请携带加盖防疫部门公章的原始预防接种记录本或卡原件及其复印
件，接种记录需有效、真实可信。
3）如果有入境国家、地区或学校关于体检及预防接种要求的表格，需一并带来，并向受理人员出示。
4）二次接种疫苗的请携带“疫苗接种或预防措施国际证书”（黄皮书）。

注：免费接种黄热疫苗，需提供前往世界卫生组织公布的黄热疫区的签证，机票复印件。
三、FOR FOREIGNERS 入境人员体检
1.Office time:8:00am-11:30am ,Monday – Friday.
2.Please make an appointment in advance,and then you can get the number according to the appointment information.If there isno appointment ,you will not be able to have a physical examination.(Please make sure all information entered by you is true and correct)
3.Please submit following documents:
♦Passport.
♦Three photos.
4. For the first-time foreign workers who want to take the physical examination free of charge is required to submit the copy of following documents:
♦ Chinese VISA .
♦ Notification letter of Foreigner's Work Permit which is issued by State Administration of Foreign Experts Affairs.
5. Foreigners who have completed the entire physical examination in your own country .Please read the following instructions carefully:
♦ Physical examination results must be provided by domestic public hospitals ,Medical examination results issued by private clinics are unacceptable.
♦ Physical examination must be done within 6 months before arrival in China.
♦Please attach all the original laboratory results.
♦Please bring along chest X-ray film and report.
♦ Chest X-ray must be done within 6 months.
♦ We reserves the right to repeat full medical check up or any specific laboratory test should there be any doubt in the medical report submitted.
6.Certificates can be obtained after three working days.
