1.体检时间：8：30-11：30（采血在10：00前采完为宜）。

2.体检手续办理：请您携带相关证件及资料到三楼大厅导诊台登记，领取体检指引单后请遵照指引单进行体检。

3.体检前24小时，应清淡饮食，禁止饮酒，正常作息；体检前12小时完全禁食禁水，体检当日清晨须空腹，慢性病（如高血压、糖尿病、心脏病）患者可正常服药或自备药。

4.需要进行尿检的受检者尽量留取晨起第一次尿样（接取中段尿样），留样时间和送检时间间隔不要超过3小时，盛放容器须用清水洗涤干净。

5.准备怀孕、孕期或哺乳期女性，请在检前主动告知登记及医护人员，不要参加X线的检查。

­

6.体检当日建议您穿宽松运动装，避免衣物有金属饰物、金属扣、亮片、玻璃珠、漆画等。您可将不便携带的物品存放在我中心三楼更衣柜中，贵重物品请您自行妥善保管。

7.应空腹检查的项目有采血、腹部超声。

­

8.体检结束后，请将指引单交至内科，经医生确认后方可离去。

2.预防接种禁忌症：

（1）急性传染病接触史，结核病，风湿病，糖尿病，严重湿疹或急慢性皮肤病，高血压，哮喘病，心脏病，肝脏疾病，肾脏病，癔病，癫痫病，慢性疾病急性发作等；

（2）发热（T>37.5℃），严重腹泻与呕吐，过敏史，免疫缺陷病，器官移植手术史，肿瘤，近一个月内免疫球蛋白使用史，正在接受免疫抑制剂治疗；

（3）异体蛋白过敏史者；

（4）孕妇、哺乳期妇女。

3.接种流感疫苗、麻风腮（MMR）疫苗等活疫苗的女性在接种后三个月内应避免怀孕，接种其他疫苗的女性在接种后一个月内应避免怀孕。

­

4.注射部位的局部皮肤应清洁，防止感染。

­

5.最好不要在空腹状态下进行，以免晕针。

­

6.接种后三天内请勿剧烈活动，不要吃酸辣等刺激性强的食物和饮酒，以免加重预防接种副反应。

黄热减毒活疫苗接种知情告知书

[接种对象]

进入或经过黄热病流行地区的人员，小于6个月龄幼儿及年老体弱者不宜接种。

[不良反应]

注射部位红、肿、痛、全身乏力，一过性热，一过性皮疹，机体免疫力低下时，接种后出现黄热病样症状。

[注意事项]

惊厥史、哺乳期妇女慎用。注意消毒剂勿接触疫苗、溶解后发现异常、浑浊的不得使用、疫苗开启半小时内用完，接种后留观30分钟，备好急救药品，使用其他活疫苗应间隔一个月，以免影响免疫效果。

[疾病知识]

霍乱是由霍乱弧菌O1或O139引起的急性肠道传染病，该病通常是轻度的，可以自身限制或亚临床的。患病严重的病人对水电解质置换疗法反应剧烈。该病主要通过食用污染的水或食物而得，人对人的传播很罕见。

­

[不良反应]

服用后观察30分钟，口服本品一般无反应，有时发生发热，轻度腹痛、荨麻疹、腹泻等，一般不需处理，可自愈。如有严重反应，应及时诊治。

­

[注意事项]

1、为取得更好效果应于餐后两小时服苗，服苗后一小时勿进食。

2、服苗后两天内忌食生冷、油腻、酸辣食品。

3、本品忌冻结，在低温冻结后不能使用。

4、胶囊经密封处理，裂开后不能使用。

5、任何急性感染或发热性疾病患者都需推迟口服本品，除非医生认为不接种会导致更大的危险。

6、由于肠溶胶囊质地较脆，应从泡罩背面沿椭圆形边缘划开铝箔，将胶囊取出，谨防胶囊破损。

7、到目前为止，任何疫苗的保护效果都不能到达100%。少数人接种后未产生保护力，或仍然发病，与疫苗本身特性和受种者个人体质有关。
